Admitting a patient to the hospice program is a simple process for the: physician, discharge planner, social worker, family member or care giver.
Just call the hospice and a staff will arrange to meet the patient/family for the explanation of the hospice benefits. Once the eligibility is determined, all hospice services requested will be provided immediately.
