Nurses from Global Hospice and Palliative Care, Inc. plan, organize and direct hospice care and build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
- Provide professional nursing care by utilizing all elements of the nursing process.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. The patient and the family are included in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsel the patient and the family in meeting nursing and other related needs.
- Provide healthcare instructions to the patient and their family/caregiver as appropriate per assessment and plan.
